Course Description: Care Management: Clinical Issues provides an introduction and overview of the care management role in the context of interdisciplinary and across systems care management for clients. Care Management: Clinical Issues focuses on skill development in managing care for the lifetime continuum of clients including disease and wellness and developmentally appropriate assessment and planning care management. The content explores the process of conducting a comprehensive assessment of the client, family and support systems, and healthcare systems to establish, implement, monitor, evaluate and document a comprehensive plan of care across healthcare systems.
